2025 Medicare Advantage plans: One network, three administrators
Our 2025 Medicare Advantage (MA) plans are all “Blue Best Life” HMO products that give members expanded network access and more affordable care. We’re not offering an MA PPO plan for this year. Here are three important points about our HMO plans:
• All of our 2025 plans use the Blue Advantage network.
If you are contracted for the Blue Advantage network, you are in-network for all of our MA HMO plans across Maricopa, Pinal, and Pima counties. The member ID prefix for all of these plans is M2K.
• Members are attributed to one of three plan administrators for claims and prior auth.
The back of the ID card displays which plan administrator handles claim filing and prior authorization:
AZ Blue
Optum Health Network Arizona (OHNAZ)
Arizona Priority Care (AZPC)
The plan administrator does not impact the plan’s network. All plans use the Blue Advantage Network.
• Referrals to specialists are not required.
Specialist referral information is not required on claims for our Blue Best Life HMO plans. However, many specialists do expect PCPs to make referrals and share information about symptoms and test results.

Optum Health Network Arizona (OHNAZ) is a separate, wholly owned subsidiary of Optum and is contracted with AZ Blue to provide utilization management and claim/payment processing services for providers and attributed members.

Arizona Priority Care (AZPC) is a separate, independent company contracted with AZ Blue to provide utilization management and claim/payment processing services for providers and attributed members. Arizona Priority Care is a service mark of Arizona Health Advantage, Inc.
