AZ Blue provider networks for the 2024 Prosano benefit plan
We are excited to be rolling out our BlueSignatureSM PROSANO (“Prosano”) benefit plans to multiple employer groups for 2024. Prosano plans include access to our Prosano HealthSM Care Centers, an integrated advanced primary care and coverage solution developed by Blue Cross® Blue Shield® of Arizona (AZ Blue).
Networks associated with the Prosano plans
Employers can offer the Prosano plan with one of these two AZ Blue provider networks (note that they are not new networks):
• Statewide/National PPO/EPO Network (most contracted providers are included in this network)
• Alliance PPO/EPO Network (exclusive network)
Members can select any provider in the network associated with their plan, including all PCPs, specialists, facilities, etc. They may also choose a Prosano Health care team provider for advanced primary care services.
ID card displays the network name + Prosano
The member ID cards will display the network name as usual, along with “+ Prosano” to indicate the additional access to the Prosano Health care team providers. Reminder: You can access most AZ Blue member ID cards via online eligibility/benefits inquiries.

You will also see the two networks listed (with the added Prosano access) in the provider directory.
Are you in-network for BlueSignature Prosano plans?
Most contracted providers are in-network for Prosano plans that use the Statewide/National PPO/EPO Network. For Prosano plans that use the Alliance PPO/EPO Network, you are in-network if you have an AZ Blue participation agreement or amendment for the Alliance PPO/EPO Network.
