Introducing Prosano Circle
Announcing our new enhancement of the AZ Blue Care Management program: 
Prosano Circle supports members in getting the care they need. 

What is Prosano Circle?
Prosano Circle surrounds an eligible member with a team of compassionate experts who will create a personalized care experience. The care team will work closely with members and their loved ones to:
• Support their immediate health needs 
• Get the most from their benefits
• Help them navigate the healthcare system
• Connect them with providers for the care they need
A Prosano Circle program does not replace a member’s PCP and specialists. It does offer members additional support for chronic disease management and care from their Prosano Health team providers at no cost.
Who is eligible?
Members with serious health conditions who meet certain criteria may be invited to participate in Prosano Circle (at no cost to them). We’re starting small for now, and we hope to include more members over time. 
Can I refer a member to Prosano Circle?
For now, please continue to refer members to our Care Management program. You can use the AZ Blue care management referral form. This will help us become aware of members who may benefit from our care coordination and care management services.
