Bill for a full lab panel when pricing for two or more individual lab codes adds up to more than the price of the panel. Lab panel codes currently affected by this change include 80047, 80048, 80050, 80051, 80053, 80061, 80069, 80074, and 80076.

The Professional Edition of the CPT^® book provides details of which individual lab codes are included in each lab panel code. 

Only the attending physician of record (or physician acting on behalf of the attending physician) may bill for professional discharge services (99238 and 99239). These codes will be denied for other clinicians involved in the discharge (other codes may be used in such instances).
Tip: Be sure to use an appropriate facility place-of-service code when billing for professional discharge services.

•	The primary cardiology procedure will be determined by relative value unit (RVU) and will receive 100% of the BCBSAZ fee schedule or billed charge (whichever is less), along with any contractual arrangements.
•	Secondary and subsequent cardiology procedures will have the technical component of the service reduced by 25%.

Note: Procedures without an assigned RVU will be ranked below procedures with an assigned RVU and are generally categorized as secondary.

•	The primary ophthalmology procedure will be determined by RVU and will receive 100% of the BCBSAZ fee schedule or billed charge (whichever is less), along with any contractual arrangements.
•	Secondary and subsequent ophthalmology procedures will have the technical component of the service reduced by 20%.

Note: Procedures without an assigned RVU will be ranked below procedures with an assigned RVU and are generally categorized as secondary.
