Help Keep Young Members on Track with Recommended Preventive Dental Care Dear Provider, This is a reminder that all Blue Cross® Blue Shield® of Arizona Health Choice (AZ Blue Health Choice) members under age 21 are assigned a dental home by six months of age or upon enrollment. To help ensure members receive the care they need, please follow the AHCCCS Dental Periodicity Schedule at every visit. The schedule outlines age-specific screenings, preventive services, and treatment requirements for members under 21. Please take the following actions: • Review and use the AHCCCS Dental Periodicity Schedule during every member visit. Tip: You can view your member roster on the AZ Blue Health Choice portal. Rosters reflect membership from the first day of the month. • Post a copy of the schedule in your office for easy reference. • Ensure all age-appropriate screenings and services are completed according to AHCCCS guidelines. • Follow the AHCCCS Dental Uniform Prior Authorization List and obtain authorization for services that require approval before treatment. • Reference the AHCCCS Dental Uniform Warranty List when determining replacement frequency for restorations and other covered services.
