New Prior Authorization Updates Effective May 1, 2026 Dear Valued Providers, We’ve updated our Prior Authorization (PA) Grids to our Medicaid, Dual Eligible (Medicaid + Medicare), and ACA Marketplace plan offerings. These updates apply to certain procedures, services, drugs, and equipment in the categories below. Review the updated codes What Providers Need to Do • Submit a Prior Authorization before services are provided for any procedure, drug, or device on the updated grids. • PA is required no matter the diagnosis or place of service. • Claims may be denied if services are provided without an approved PA.
