Blue Cross Blue Shield of Arizona Health Choice (AZ Blue Health Choice) is committed to fostering strong provider partnerships and continuously improving the service and support we provide to our network. As part of this commitment, we will be conducting our annual Provider Satisfaction Survey. The survey results will help guide future improvements and enhance the overall provider experience.

Selected providers and provider office staff may receive survey invitations through a combination of email, mail, and telephone outreach during the survey period. Enhanced outreach efforts have been incorporated for the 2026 survey cycle to encourage participation and increase response rates.
