AHCCCS has launched the AHCCCS Solutions Center, a centralized platform designed to streamline communication between providers, health plan staff, and AHCCCS.
With the Solutions Center, you can:

MOC Training Reminder
2026 Annual Model of Care (MOC) Training is available for Health Choice Pathway (HMO D SNP) providers. The Model of Care supports care coordination and improved outcomes for members with special health care needs.
Key reminders — MOC Training:

Is open annually from January 1st to December 31st
•
Includes contracted and non-contracted providers, including the clinical, administrative, and support staff
•
Contracted providers are considered qualified to deliver services aligned with DSNP members' specialized needs
