AZ Blue members can access digital images of their ID cards and send them directly to your office. They may also carry paper ID cards with them. In any case, real-time eligibility checks will get your office staff everything they need to care for our members.
Please note: Digital, paper, or emailed card images are VALID and should be honored.

July prefix updates
You can download our updated prefix list and view the refreshed table of prefix replacements at azblue.com/prefix. Here are the highlights:
New prefix replacement: XBO → HMO
Some plans with the XBO prefix will begin using the new HMO prefix on 7/1/25. Tip: Use the correct prefix for the date of service when validating eligibility and submitting claims.
Discontinued prefixes: XBD, LNT, and KGM
(will remain active for previous dates of service) Members with the KGM prefix started using the AOZ prefix on 7/1/25.
Prefixes for “Strategic Hub” plans: AOZ, AZI, AZK, and IVR
These plans are administered in partnership with another Blue Plan (Blue Cross Blue Shield of South Carolina). Learn More.

The CMS risk audits have begun for 2018 dates of service, soon to be followed by the audits for 2019-2023. Look for record request faxes (10-day turn-around time). Learn more about these CMS risk audits. Note: CMS requires medical records for Medicare members to be retained for 10 years.
Tip to avoid claim denials: Make sure your claim/billing team includes the authorization number in applicable claims. For more tips, see our checklist for clean claims.
