When our members need care, they turn to YOU for help.
Need to validate eligibility? Here are 5 accepted ways to get the current member ID.
AZ Blue ID Card
Most members will receive a card by the first day of their new plan year. For renewing members, last year’s card also works.
Printed ID Card
Cards (front & back) printed at home can be accepted.
Digital ID Card
Members can log in to their account and email a PDF or text a screenshot of their card (front & back).
Digital Credentials
Members can share identification credentials through their mobile “wallet” so you can validate eligibility.
Member Info
Members can share their name, member ID, and date of birth so you can validate eligibility.
Need more help?
If you receive an error message when validating eligibility, please do not turn the member away. Please email us at providerhelp@azblue.com for help (most responses received within 2 hours).
